Camp Staff Acknowledgment Form
This Agreement is made between I, the undersigned, and United Cerebral Palsy of Greater Sacramento, Inc., also doing business as UCP of Sacramento and Northern California (hereinafter referred to as UCP of Sacramento and Northern California).
A. News and Photo Release
I grant UCP of Sacramento and Northern California permission to take or have taken still photographs or videos and films, including television pictures of me, and use them without restriction.
I further consent and authorize UCP of Sacramento and Northern California, it’s advertising agencies, new media, and any other persons interested in and its work, to use and reproduce the photographs, films, and pictures and to circulate and publicize the same by all means including without limiting the generality of the foregoing newspapers, television media, social media, brochures, pamphlets, instructional material, books and clinical material.
With respect to the foregoing materials, no inducements or promises have been made to me to secure my signature to this release other than the intention of UCP to use or cause to be used such photographs, films, and pictures for the primary purpose of promoting and aiding UCP and its work.

C. Early Departure Transportation Responsibility
By signing this form, I acknowledge and agree that if I must leave UCP Camps and/or events prior to the scheduled end date for any reason, I am solely responsible for arranging and securing my own transportation from the camps and/or events. I will not expect transportation assistance from UCP or Sierra Nevada Journeys (SNJ) personnel for any unscheduled departure.
By signing below, I acknowledge I have read the "Camp Staff Acknowledgment Form", including the "News and Photo Release", the "Release of Liability and Informed Consent", and the "Early Departure Transportation Responsibility", and I fully understand its terms. Additionally, I understand that I am giving up substantial rights, including my right to sue and be photographed.
